We are open for walk-in visits as follows:
Tuesday evenings: 6:00pm - 8:00pm
Saturday mornings: 9:00am - 11:00am
Saturday mornings: 9:00am - 11:00am
Any adult resident of Ashland Area who meets our criteria may be seen during these hours by one of our volunteer physicians. We see patients on a first-come, first-serve basis and make every effort to see all who come in during these times.
We serve patients with any general medical ailment such as diabetes, high blood pressure, depression, abdominal pain, emphysema, asthma, headaches, sore throat, cough, and we can also perform general physical exams. If the patients' needs cannot be met, we will try to arrange for an appropriate referral. We do not perform PAP tests; for those, we can make recommendation and referral.

How Much Does It Cost?
The charge is $25.00 per visit. Patients will, however, be seen even if they cannot pay at the time of their visit. If they cannot afford to pay for the visit, we ask for patients to 'Pay it Forward' with an act of community service to others. We ask for 4 hours of volunteer service per each clinic visit. Patients will be given a voucher to be returned to ACHC with the number of hours and service performed.
There is the option to pay an amount based on a sliding income scale. This payment amount is based on family size and income, according to the Federal Poverty Guidelines, not to exceed more than $50.00. More information is available at the clinic.

Medication Policy
It is the policy of ACHC that NO controlled medications are to be prescribed by our medical providers. This policy is necessary because we are unable to provide adequate oversight and meet legal requirements for prescribing. The medicines included in this policy are:
1) All narcotics such as Vicodin, Percocet, Oxycodone, etc.
2) All benzodiazepines such as Xanax, Ativan, Klonipin, etc.
3) All stimulants such as Ritalin, Adderall, etc.
4) All weight loss products such as Phentermine.
If it is determined that a controlled substance is needed to manage the patient's condition, we will try to arrange for an appropriate referral.
Our Pharmacy
Our pharmacy is stocked with a number of medicines which include purchased medications, medications bought with grant money, unused medicines from local Extended Care Facilities, and sample medicines from the manufacturers. These medications are provided for free. If a medication is not available in our pharmacy, several local pharmacies have agreed to supply patients of ACHC with prescribed medications at a reduced price. We will try to use the lowest priced medicine that is appropriate for the patient's condition. Finally, for long-term medicines that are more expensive, patients will be directed to our Patient Medication Assistance Program (PMAP), which will help out with necessary paperwork to obtain the medicine from the manufacturer.

At a lab or hospital of patient choice. A referral will be given and most often financial forms are available at the Health Center for their Financial Assistance Program. Be sure to ask about this program when you go to the hospital for any reason. Physician fees, such as those charged for reading an X-ray, are not automatically reduced. You will be responsible for these payments. Another option is to pay cash.
What do I do if I have medical questions when the Health Center is not open?
If you have an emergency, you will need to a Hospital emergency room. If you need to be admitted to the hospital, the hospital physician will care for you. You will probably be billed for this service. If your question can wait, you can either call the Health Center at 419-903-0475 or leave a detailed message and our nurses will return your call during office hours.
How will I find out my test results?
Results will be sent to the Health Center and you will be notified by phone or in writing. For this reason, it is important that we have a current phone number and address in your chart. If you do not hear from the Health Center, feel free to call. You can also pick up the results at the clinic the week after your tests are done.
Are there other medical supplies available at the Health Center?
Yes, we provide some diabetic and dietary supplies as well as durable medical equipment such as walkers, crutches, nebulizers, etc. free of charge. We welcome donations.
